I, Ms. Loriefae Mae M. Polon, Teacher III at Venancio Trinidad Sr. Memorial School,
Talisay District, Division of Batangas Province, submits this letter of request for an original
copy of my appointment to your good office.

Last November 22, 2022 was the date of signing for my Teacher III appointment at the
Division office of Batangas. However, upon the occurrence of the event, the salary that was
indicated in the said document was deemed to be incorrect, as it reflects a salary which is
different from the 4th tranche of standardization law, amounting only to 28,276 pesos
instead of 31,320 pesos.

Hence, we were advised that your good office shall release the correct and original copy of
the document at a later date. With that, I am hoping that your humble office may furnish the
requested documents in your most available time.
