AFFIDAVIT OF NO EMPLOYMENT
(For the purpose of Correction of Entry in the Birth Certificate)

I, MARTIN PALEG DONIEGA, JR., minor (11 years old), Filipino, and
a resident of Carig Norte, Tuguegarao City, Cagayan, after having been duly
sworn to in accordance with law, do hereby depose and state:

1. That as of the present time, I am not employed in any capacity at

any public or private agency or person;

2. That I have applied for the correction of entry in my Birth

Certificate, specifically, Sex/Gender in the City Civil Registrar’s
Office of Tuguegarao City, Cagayan;

3. That I am executing this affidavit to attest to the truthfulness of the

foregoing statements and to support my application for the
correction of entry in my Birth Certificate in the City Civil
Registrar’s Office of Tuguegarao City, Cagayan.
