DATA PRIVACY CONSENT

In compliance with the Data Privacy Act of 2012, I, ____________________


Lloyd Molo allow
Outsourced Quality Assured Services, Inc. (“Outsourced”) to store and collect my
personal information for purposes of:

[] Employment application;

[] Employment;

[] Supplying equipment, devices, assets; or

[] Procurement of services.

As such, I agree and authorize Outsourced to:

1. Collect, store, or continue to use my personal information for the above-


stated purpose;
2. Retain my information for a period of not more than six (6) months from
expiration of my contract, unless in cases of separation of employment

which shall be not more than five (5) years for purposes of record-keeping,
or at such time that I submit to Outsourced a written cancellation of this
consent, whichever is earlier. I agree that my information will be
deleted/destroyed/removed after this period;
3. Share my information to affiliates and necessary third parties for any
legitimate business purpose. I am assured that security systems are
employed to protect my information;
4. I acknowledge my rights under the Data Privacy Act, such as, but not
limited to right to object, remove, edit, access, and be informed of any
security breaches involving my personal data.

I also acknowledge and warrant that I have acquired the consent from all parties
relevant to this consent and hold free and harmless and indemnify Outsourced
from any complaint, suit, or damages which any party may file or claim in relation
to my consent.
