PAKYAW CONTRACT AGREEMENT

This agreement made and entered by and between SHARYLL R. STO. TOMAS, single, of legal age
and the School Head of CALAMBOG ELEMENTARY SCHOOL, herein after referred to as the FIRST PARTY

AND

GIL D. DOBLON, married, of legal age and with residence and postal address at SAN FERNANDO,
CAMARINES SUR herein and referred to as the SECOND PARTY.

WITNESSETH

THAT THE FIRST PARTY:

1. Will implement its school project under the 2023 MOOE for Repair of Ceiling, the Program of Works
(POW) is hereto attached and forms integral part of this contract.
2. Will provide the necessary materials needed for the school project and will be delivered on site.
3. We will pay the Second Party the sum of Eleven thousand five hundred thirty-eight pesos within three
(3) days after the completion of the project.

THAT THE SECOND PARTY:

1. Will provide the labor necessary to carry out the implementation of the project in accordance with the
Program of Works (POW).
2. Will finish the project within ten (10) calendar days upon approval of this contract.
3. Will collect full payment for labor per item number 3 above.
4. Will undertake corrective works within the year from the date of turnover at no extra labor charge as part
of defect liability period.

In witness hereof we have here unto set our hands this 19th day of June, 2023 at San Jose East,
Canaman, Camarines Sur.
