PAKYAW CONTRACT AGREEMENT

This agreement made and entered, by and between EDNA F. ALADANO, of Bagong
Sirang, Pili, Camarines Sur married of legal age and the School Head/Principal of GOV.
MARIANO E. VILLAFUERTE HIGH SCHOOL, herein after referred to as the FIRST PARTY;

AND

GLENMON A. MARTOS. of legal age, married, a with residence and postal address
Banga Tinambac Camarines Sur herein referred to as the SECOND PARTY.

WITNESSETH

THAT THE FIRST PARTY;

1. Will implement its school project under the Maintenance and Other Operating
Expenses (MOOE), the program of work (POW) of REWIRING is hereto attached
and forms integral part of this Contract.
2. Will provide the necessary materials needed for the school project and will be
delivered on site.
3. Will pay the second party the sum of Ten Thousand Five Hundred Pesos only, (Php
10,500.00) within _15_ days after completion of the project.

THE SECOND PARTY;

1. Will provide the labor necessary to carry out the implementation of the project in
accordance with the approved program of works (POW) OF REWIRING
2. Will finish the project within 15 calendar days upon approval of this Contract.
3. Will collect full payment for labor per item number 3 above.
4. Will undertake corrective works within one year from the date of turnover at no extra
labor charge as part of defect liability period.
