PAKYAW CONTRACT AGREEMENT

This agreement made and entered by, and between GIL B. VIVO, married of legal age and the ESP-
I/SCHOOL HEAD OF CAGAYCAY ELEMENTARY SCHOOL herein after referred to us the FIRST PARTY.

AND

___________________________ of legal age, married with residence and postal address at CAGAYCAY,
GOA, CAMARINES SUR, herein referred to as the SECOND PARTY.

WITNESSETH

THAT THE FIRST PARTY;

1. Will implement its school project under the School Based Repair and Maintenance Scheme
(SBRMS), the Program of Work (POW) is hereto attached and forms integral part of this contract.
2. Will provide the necessary materials needed for the school project and will be delivered on the
site.
3. Will pay the Second Party the sum of ___________________________________________,
(Php._____________) within (____) days after the completion of the project.

THAT THE SECOND PARTY

1. Will provide the labor necessary to carry out the implementation of the project in accordance
with the approved Program of Works (POW) contract.
2. Will finish the project within Twelve (12) Calendar Days upon approval of this Contract.
3. Will collect full payments for labor per item number 3 above.
4. Will undertake corrective works within one (1) year from the date of turnover at no extra labor
charge as part of defect liability period.
