    EXCUSE LETTER FOR ABSENCE

    Good day Ma’am Pablo,

    I, Mark Justin A. Lasam of 2F-PH, would like to ask for your kind consideration since I
    have not been able to attend classes for the past days specifically April 20, 2023 – April 29,
    2023. I have been diagnosed with acute gastroenteritis with moderate dehydration. I was
    admitted at the Perpetual Help Medical Center – Las Pinas and I had to take time off to
    recover since I have had continuous episodes of vomiting after I was discharged. I have
    already sent a softcopy of my medical certificate through email. I hope for your kind
    consideration in allowing me to make-up for my absence during our group’s presentation
    regarding Chiropractic Therapy.
