“RESOLUTION AUTHORIZING BARANGAY TREASURER IRIS D. PLAZA, CNSPMC

CHAIRPERSON MARIA CARIDAD IMPORTANTE AND AREA COORDINATOR JOMIL C.
ACUÑA TO OPEN COMMUNITY CHECKING ACCOUNT AT THE LAND BANK OF THE
PHILIPPINES – NAGA EVANGELISTA BRANCH FOR THE IMPLEMENTATION OF DSWD
KALAHI CIDSS: PMNP”

WHEREAS, Barangay San Pantaleon has been identified as site for the implementation of
KALAHI CIDSS: PMNP funded by the National Government in a form of a grant and is being
implemented thru the Department of Social Welfare and Development (DSWD) where it is in the
Sub-Project;

WHEREAS, Barangay San Pantaleon is one of the beneficiaries that will be given grant fund by
the DSWD KALAHI CIDSS: PMNP;
WHEREAS, Land Bank of the Philippines is the authorized bank of the Project in order to
transfer the community grant;
WHEREAS, the Sangguniang Barangay Authorized Barangay Treasurer IRIS D. PLAZA
CNSPMC Chairperson MARIA CARIDAD IMPORTANTE and Area Coordinator JOMIL C.
ACUÑA as signatories to the checking account with the name of DSWD KALAHI- CIDSS:
PMNP BRGY SAN PANTALEON;
NOW THEREFORE, on motion made by Barangay Kagawad
____________________________, duly seconded by Barangay Kagawad
____________________________ and with the concurrence of all the members of the
barangay council present, be it;
RESOLVED, as It Is hereby resolved to authorize Barangay , CNSPMC Chairperson MARIA
CARIDAD IMPORTANTE and Area Coordinator JOMIL C. ACUÑA of Barangay San Pantaleon
Magarao. Camarines Sur to open a community checking account and signatories of the
checking account at the Land Bank of the Philippines- Naga Evangelista Branch for the Sub-
Protect Implementation under DSWD KALAHI CIDSS: PMNP;
RESOLVED that copies of this resolution be furnished and submitted to all concerned office
concerned further information, guidance and appropriate action.
