PruLink Health Extend is a regular-pay variable life insurance Proposed Insured : JOSEFINA DIAZ ECHIPARE
product that provides living, disability and death benefits. The Age : 55
living benefits are the partially and fully withdrawable values

which are taken from the policy's share in the separate
fund(s). The Accelerated Total and Permanent Disability Regular Annual Premium : PHP 24,000
benefit accelerates a percentage of the sum assured. The
Sum Assured : PHP 360,000
death benefit is the total of the sum assured and the policy's
full withdrawal value. Optional benefits can be included in Accelerated Total and
Permanent Disability Benefit : PHP 300,000

Any policy debt shall be deducted from the death benefits/ supplemental benefits.
When the fund value is insufficient to pay the policy fee, insurance charge, rider charge and other applicable charges
(including any policy debt), the policy automatically terminates and all benefits end. In such an event, the remaining balance of
your units which are no longer sufficient to pay off outstanding charges will just be refunded to you.
