AFFIDAVIT OF SOLO PARENT

I, Norme Tuballas, 61 years old, a widow, Filipino, and a resident of Brgy. Dale San Roque
Northern Samar, after having been duly sworn to in accordance with the law do hereby
depose and state:
1. I was married to Arturo Tuballas on April 18, 1984.
2. Out of the said marriage, I gave birth to several children. Currently, Mary Cris Tuballas
is dependently living with me.
3. That my husband Arturo Tuballas died on July 26, 2017.
4. Since the death of my husband up to now, I have been the only one who is in custody
of our children and supporting our children in their education and daily basic needs.
5. I am executing this affidavit to declare that I am not cohabitating with another partner
or co-parent and have the sole parental care and support of my children.
IN WITNESS WHEREOF, I have hereunto set my hand this 9th day of September
