RELEASE, WAIVER AND


QUITCLAIM OF RIGHTS

I, JOEL RONIE MAYONO, of legal age, single, Filipino citizen and with residence and postal
address at Paradahan II, Tanza, Cavite, after having been duly sworn to according to law depose and say;

1. That I am the registered owner of a proposed two (2) storyx commercial building situated at
Brgy. Paradahan, Municipality of Tanza, Province of Cavite, Island of Luzon, covered by
Transfer Certificate of Title No. 057-2022073180;

2. That the above-mentioned property is subjected for road legal easement (setback) a total of five
meters (5 m) by the Department of Public Works and Highways (DPWH);

3. That I undertake as the sole owner of the abovesaid property, I fully waive all my rights regarding
the five meters (5 m) setback of the above-mentioned commercial building to Department of
Public Works and Highways (DPWH), whereas, in the event that the DPWH will require the
easement, I am giving them full authority to remove and demolish the part of the aforementioned
building subjected for easement and that under laws and rules set out by Power of Eminent
Domain of the Republic of the Philippines, the DPWH shall compensate the me, my heirs,
assigns and successors and alike, in fair market value of the above-mentioned commercial
building;

4. That we are executing this affidavit to attest to the truthfulness of the foregoing facts and to use
the same to whatever legal purposes it may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this April 20, 2023 at Trece
Martires City, Cavite, Philippines.
