SWORN AFFIDAVIT

I, RICHARD CERVA GOMEZ, of legal age, single, Filipino citizen and with residence and
postal address at Block 8 Lot 4 Talisay Street, St. Joseph Homes, Brgy. Inocencio, Trece Martires City,
Cavite, hereby state and depose that;

1. That I am the registered owner of a Globe Number and a Gcash Account with SIM Card No.
0966-403-9389;

2. That sometime this January, 2023, the aforesaid SIM number got deactivated without my
knowledge, apparently, the same was deactivated due to prolonged insufficiency of balance,
hence, this affidavit;

3. That I intend to transfer the remaining balance due to my Gcash Account to my another Gcash
Account with Gcash Account No. 0906-888-9671;

4. That I attest for the truthfulness of the above-mentioned facts.

I HEREBY DECLARE UNDER PENALTIES OF PERJURY THAT THE FOREGOING
