I, AILEEN LUPERA ORTIZ, of legal age, Filipino citizen, with business address at 024
Governor’s Drive, Brgy. Cabuco, Trece Martires City, Cavite, Philippines, after being sworn in
accordance with law hereby depose and say;

1. That I am the registered owner of a business under the trade name AMIGAS
TRAVEL AND TOURS located at 024 Governor’s Drive, Brgy. Cabuco, Trece
Martires City, Cavite, Philippines;

2. That I have a Gross Income for year 2023 amounting Fifty Thousand Pesos
(₱ 50,000.00);

3. That I am executing this affidavit to attest the truthfulness of the foregoing

declarations under penalties of perjury, moreover to further serve all legal
intents and purpose.
