sworn to according to law depose and say;

1. That I am the owner of the business name registered ORIJ-IN CONSUMER GOODS
TRADING located at BES Bldg. 51 Bucal, Sampaloc II, Dasmariñas City, Cavite, with
Certificate No. 3189789 and valid from September 17, 2021 to September 17, 2026
prescribed by the Department of Trade and Industry (DTI);

2. That I hereby cancel said business name registered ORIJ-IN CONSUMER GOODS
TRADING, with no financial obligation;

3. That I am executing this affidavit to attest to the truthfulness of the above statements.
