AFFIDAVIT OF LOSS

I, REYMOND JAY RIAS BOY, of legal age, Filipino citizen, and with
residence address at # 005 Pag-asa II, Imus City, Cavite, after having been sworn to in
accordance with law hereby depose and state that:

1. I am a duly registered subscriber of a Globe Sim with No. 0916-6419762;

2. Unfortunately, the SIM card of my aforementioned subscription has been

missing and the same was lost on May 11, 2021 and cannot be located
despite diligent efforts, hence, the same is deemed lost;

3. I hereby executed this affidavit in order to attest to the truthfulness of the

foregoing facts and for all legal intents and purposes this may serve.
