Today, March 30, 2023, I arrived at our site early in the morning at exactly 7:00 a.m.

Later, the
barangay health center was open, and we helped them clean the area by cleaning the floor with a
soft broom. Later on, we did a consultation with a male patient who was 62 years old. I took his
blood pressure, which was 110/70 mmHg, which is in the normal range. Another male patient
came for consultation. I rested him because the weather outside was very hot. A minute later, I
asked him about his previous blood pressure before I was going to take it, but he didn’t
remember. I also asked if he felt the need to pee or not, and he said not. So I started to take his
blood pressure, which was 180/100 mmHg, which is higher and will be considered hypertension
stage 2, which needs immediate medical attention. We informed them to take rest, and he felt like
urinating, after he passed out. He came back to take a rest, and I took his pulse rate, which was
95 bpm. I took his blood pressure again, and it was the same. We asked him what his medication
was; as per the patient, he was taking Angimax 35 mg tablets twice a day as prescribed by his
physician for his heart problem. Unfortunately, his medication for hypertension, losartan 50 mg,
had been discontinued 3 years ago.

We health-educate him that any medication prescribed as maintenance by his physician must be
regularly taken in order to prevent increased blood pressure and other unexpected emergency
health problems that may arise. He must go to his physician for an immediate checkup. Due to
the increase in his blood pressure, they gave him a referral consultation form, and he must go
today to barangay San Isidro 1 for further assessment by the physician. We educate the patient
about his lifestyle so that he can change it, such as his drinking of alcohol. I asked him also what
he had for breakfast today, and he said fried eggs, so I educated him to avoid too much salt and
instead eat more vegetables and fruits and do regular exercise.

Lately, we don’t have any patients2, so our CI did a written and oral quiz. I think I passed his
written quiz about family planning and immunization. Then followed the oral quiz. Where I
answered only the first question about what are the services offered in this barangay health
center. On two of the questions, I really didn’t hint at the specific answer being asked. He asked
about what the main purpose of family planning is; the correct answer is all about birth spacing
or interval. The third question about why prenatal checkup is very important during the first
trimester of pregnancy is that it is important to monitor and determine any health problems with
the mother if she has anemia by doing blood tests and urine tests to determine also if she has a
possible UTI or STD that is high risk for the development of her baby, in order to assess and
monitor what are the effective interventions needed, such as medication, to prevent any
complication for both the mother and her baby. Therefore, anything the mother ingests that
affects her health status will affect the development of her baby, as the baby receives the
nutrients from the mother..
