JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, ERWEL JOHN LOYD GUMISONG, Filipino of legal age, and a resident of Talairon,
Oroquieta City, Misamis Occidental and REXELL ANN ANTIPUESTO, Filipino of legal age, and a
resident of Talairon, Oroquieta City both under oath, hereby depose and say:

1. That we personally know ELSA DOMA-UG CARABAL who is born on May 23, 1947,
in Dampalan, Lopez Jaena, Misamis Occidental. Copies of her Birth Certificate and
her Marriage Contract are hereto attached and marked as Annex A and B;

2. That in her SSS account her name is erroneously reflected as ELSA DOMA-OG
CLARABAL with a date of birth on May 27, 1947 when it should have been ELSA
DOMA-UG CARABAL born on May 23, 1947;

3. That ELSA DOMA-OG CLARABAL born on May 27, 1947 and ELSA DOMA-UG
CARABAL born on May 23, 1947 refer to one and the same person;

4. That her father’s and mother’s last name was wrongfully entered as SENON DOMA-
OG and SEBASTIANA when it should have been CENON DOMA-UG and SEBASTIANA;

5. That SENON DOMA-OG and CENON DOMA-UG refer to one and the same person, the
same way with her mother SEBASTIANA and SEBASTIANA;

6. That the reason why she and her husband use the last name CLARABAL is because
most of her husband’s cousin use the said last name;

7. That we are not related by affinity or consanguinity to the above mentioned person;

8. That we are executing this affidavit to attest to the truth of the foregoing facts for
whatever legal purpose this may serve.

IN WITNESS, we have set our hands this 11th day of December 2020 at Oroquieta City,
