SUBJECT: Progess Report re Shooting Incident that Resulted to the Death of HARVEY LAFORTEZA y

CERA

DATE: 20 NOVEMBER, 2021

1. Reference Memorandum No. 148-DC-0427 From OIC Dagupan City Police Station Dated October
10, 2021 with subject same as above.

2. This pertains to the shooting incident that transpired at about 10:00 AM October 10, 2021 at
Brgy. Nagsaing, Calasiao Pangasinan. Initial Investigation conducted disclosed that HARVEY
LAFORTEZA y CERA, 32 years old, married, laborer, and a resident of brgy. Nagsaing, Calasiao
Pangasinan, was shot by suspect HAROLD AUSTRIA y ESCOSIO, 43 years old, married, Tricycle
Driver, and a resident of No. 120 Lasip Chico, Dagupan City. As a result, thereof, the victim
sustained multiple gunshot wounds in the different parts of the body and was rushed to
Nazareth Medical Center for medical treatment but was pronounced dead on arrival (DOA) by
the attending physician thereat.

3. The Incident was witnessed by ascertain PAUL LEE DEVERA, 34 years old resident of same place.
Personnel of Dagupan City PS responded to the suspect and was arrested at his house during
hot-pursuit operation conducted, but the weapon used in the commission of the crime was not
recovered and he is now under the custody of this station for investigation and proper
disposition. The cadaver of the victim was moved to Crime Laboratory Office for autopsy
examination.
