assigned teachers.

3. Get your loading slip from the assigned teacher

4. Fill up the loading slip without erasures and make
sure that the following fields should be correctly
filled-out.
- Name (Surname, First Name, M.I.)
- Program/Course
- Semester / Academic Year
- Course Code
- Course Title
- Units
NOTE: Your Prospectus is your guide as to what
course/subject you are going to enroll for this 2nd
semester of 2022-23

5. Proceed to your respective coordinators to verify the

filled-out loading form and sign the enrollment log
form.
6. Return your prospectus and loading-slip student copy
to the assigned teacher
