AFFIDAVIT OF LOW INCOME

I, GRAZIELLE A. ANDO, Filipino, of legal age, married, and a resident of Purok

Patag, Barangay Dao, Pagadian City, Zamboanga Del Sur, after having been sworn in
accordance with law hereby depose and state:

1. THAT, I am the same GRAZIELLE A. ANDO who is executing this affidavit;

2. THAT, I am only a “barangay staff” with low-income family whose annual income is only
TWO HUNDRED NINETEEN THOUSAND TWO HUNDRED FIFTY-TWO
(P219,252.00) PESOS ONLY;

3. THAT, I am the mother of GRAYZIE FHYL A. ANDO who will be taking a scholarship
at JH CERILLES STATE COLLEGE;

4. THAT, I cannot financially support my daughter in her tertiary education;

5. THAT, I am executing this Affidavit to attest to the truth of the foregoing facts and to use
the same for whatever legal purpose it may serve.
