Chandigarh - 160036 do hereby solemnly affirm and declare as under:

1. That I am citizen of India.

2. That I am bonafide resident of House no 207, Sector 38-A, Chandigarh -
160036.
3. That I have applied for an Education Loan from HDFC Bank for Gaurav Malik
for MBBS from PGI, Rohtak.
4. That neither I nor any other member of my family has applied for/availed any
type of Loan for any member of the family from any Bank/Financial Instituition
other than those declared in the loan application form. If the information
furnished by me is found to be incorrect, bank is authorised to take appropriate
action as deemed to be if necessary.
