Tehsil Pattan Distt: Baramulla Kashmir, do hereby undertake as under:-

1. That I am consumer of Electric Department vide Installation No:6953, No JD No 0213040000353

2. That I intend to disconnect the said installation on my own will and responsibility that I have paid all
dues till date for the aforesaid installation number.

3. I further declare and undertake, if in future I will use the electricity without agreement, and if the
concerned authorities found that I have used the electricity illegally, then I shall be liable to repay
the all fee dues, and the Deptt: is authorized to take legal action against me as deemed fit. I shall not
claim for installation No / agreement in future till next/

4. If the above contents of this affidavit are found false/incorrect, I shall be myself personally
responsible and shall be punished under the law.

Baramulla Kashmir, do hereby undertake as under:-

1. That I am a permanent resident of State Jammu and Kashmir hailing in Village Ganjipora
Buran Tehsil Pattan.

2. That I am working in Power Development Department as Line Electrician as Labour on

daily wages.

3. That I am myself personally if anything happens during the work, and I shall not claim for
any relief from concerned department at any stage.

4. If the above contents of this affidavit are found false/incorrect, I shall be myself personally
responsible and shall be punished under the law.
