DISCONNECTION AFFIDAVIT

I, NIRMALA DEVI WIFE OF SHRI BHARAT BHUSHAN RESIDENT OF H.NO. D-55/A, (G-
55/1), JAGATPURI, DELHI-110051, do hereby solemnly affirm and declare as under :-
1. That I am the sole user if connection No. 100760096, which is registered on the
name of NIRMALA DEVI have been using through the already installed K.No. as
above. I want to remove the said connection. I deposited all dues charges upto
dates & I agree to pay all dues losses or expenses against said connection in
future if any and fully responsible for the legal dispute against the connection and
recovery of the said connection if any can be recovery for the connection.
