I hereby willingly and voluntarily give consent for the participation of my son/daughter in joining the 11
Madigan Farewell Party to be held on June 29, 2023, from 10:00 AM – 4:00 PM, at the Ultra Winds
Mountain Resort. By signing this form on June 28, 2023 (Date) I confirm that I have fully informed myself
of the content of the event and this Parental Consent.

I have considered the benefits that my son/daughter will derive from his/her participation. Moreover, I
understand that this is a student/class-initiated event, and will take place outside the school premises,
where unforeseen circumstances may happen, as such I will not hold the school, organizers, nor the
moderator liable for incidents before, during, and after the event.
