Parent’s/Guardian’s Consent Form and Waiver

I herewith declare that I am the parent or legal guardian of the contestant, a minor, and that I have the
legal authority to sign this authorization in his/her name.

By affixing my signature below, I confirm that I have read and understood this Consent Form and Waiver
and hereby permit my child to participate in the 𝑆 3 𝐹𝑒𝑠𝑡𝑖𝑣𝑎𝑙 to be held on February 03, 2023, hereinafter
referred to as the Competition. Furthermore, I and my child agree to be bound by the rules/mechanics of
the Competition and by this Consent Form and Waiver.

The Practice hours can be held during Math, English, and MAPEH classes. Practices during off class
hours (4-6PM) can and shall be done only in MSHS covered court or grounds with the presence of
assigned teacher-supervisors.

Schedule of per level practices are as follows:

Grade 7 & 9 Tuesday

Grade 10 & 11 Friday
Grade 8 & 12 Thursday
All Saturday by Schedule

SATURDAY SCHEDULE
Grade 7 & 8 08AM – 10AM
Grade 9 & 10 10AM – 12NN
Grade 11 & 12 01PM – 03PM

I understand and agree that participation is voluntary and that Muntinlupa Science High School, the
MSHS Mathematics Department, the MSHS English Department, the MSHS MAPEH Department, and
the organizers of the Competition, hereinafter referred to as the Organizers, will not be liable for any
injuries and untoward incidents sustained during the Competition and its Practice hours. I also understand
that my child cannot participate in Practice hours and the Competition without a signed
Parent’s/Guardian’s Consent Form and Waiver.
