Note: Setting up your Computer Name is important. This will serve as the

This Memorandum of Agreement made and executed between: Don Vicente Rama
Memorial National High School a Technical Vocational Institution, duly organized and
existing under Philippine Laws with office business address at represented herein by
School Coordinator Mr. Luis C. Gerunda herein after referred to as DVRMNHS;

An enterprise duly organized and existing under Philippine Laws with office business
address at represented herein by its admin office, Leah R. Ornopia and herein referred
to as the TMJ Training Center,Inc.
WITNESSETH:
The parties hereby bind themselves to undertake a Memorandum of Agreement for the
purpose of supporting the Supervised Industry Learning (SIL) for Learners under the
following terms and conditions:
1.The WI shall be responsible for briefing the Learners as part of the TVI`s TIP and
Job Induction Program.
2.The TVI shall provide the learner undergoing the SIL with the basic orientation on
work values, behavior, and discipline to ensure smooth cooperation with the
PARTNER ENTERPRISES.
3.The TVI shall issue an official endorsement vouching for the well- being of the
Learner which shall be used by the PARTNER ENTERPRISES or processing the
learner`s application for SIL;
4.The TVI shall voluntarily withdraw a Learner who is found to misbehave and/ or
act in defiance to existing standards, rules, and regulations of the PARTNER
ENTERPRISE and impose necessary TVI sanctions to the said learner;
5.The WI shall encourage its LEARNER to complete the duration of his her SIL;
7.The TVI has the discretion to pull out the Learner if there is an apparent risk and/
or exploitation on the rights of the Learner;
8.The TVI shall ensure that the Learner has an on- and off the campus insurance
coverage within the duration of the training as part of their training fee.
9.The TVI shall ensure Learner shall be personally responsible for any and all
liabilities arising from negligence in the performance of his/ her duties and
functions while under SIL;
PARTNER ENTERPRISE:
10. The PARTNER ENTERPISE may grant certain incentives (i.e. food,
accommodations, transportation, allowance) to the Learner in accordance with the
PARTNER ENTERPRISE`S existing rules and regulations provided it will be
beneficial for them;
11. The PARTNER ENTERPRISE have the option to hire or employ the Learner,
provided that the Learner had finished complying with the duration of SIL;
12. The PARTNER ENTERPRISE shall see to it that the LEARNER shall not be
assigned to other work not related to the competencies specified in the Training
Plan;
15. No other expenses shall be charged to the learner in the conduct of SIL;
16. The PARTIES shall exert efforts to ensure that the training shall be conducted in
accordance with the provisions of the Labor Code and other pertinent laws. It shall
17. Any violation of the foregoing covenants will warrant the cancellation of the
Memorandum of Agreement by the non- breaching party within thirty (30) days
upon notice to the other party;
This Memorandum of Agreement shall become effective upon signature of both parties
and implementation will begin immediately and shall continue to be valid hereafter until
written notice is given by either party thirty (30) days prior to the date of intended
termination. In witness where of the parties have signed this Memorandum of Agreement
at CHEF`S TABLE this 1st day of September 2023.

personally appeared and with sufficient proofs of identification, known to me to be the
same persons who executed the foregoing instrument and they acknowledged to me that
the same is their free will and voluntary deed and that the ENTERPRISEs herein

GENERAL INTERPRETATION:
The ratings provided by the five raters (ranging from 4.2 to 5)
consistently reflect a high level of satisfaction with the subject being
evaluated. Rater 1 has a score of 4.2 which means very good. Raters
2 and 5 all assigned a score of 5, which indicates that they found the
subject's performance or qualities to be exceptionally strong or
outstanding. Raters 3 and 4, with scores of 4.6, also contributed to
this positive evaluation, demonstrating a consistently positive
perception across the board. The overall average rating of 4.68 further
supports the notion that the subject's performance was not only
commendable but also reached a level that is considered "outstanding.
This collective assessment suggests a strong consensus among the
raters about the satisfaction of the training that they have undergone.
RECOMMENDATION:
In view of the data shown, it is highly recommended that Don Vicente
Rama Memorial National High School continue their partnership in
foregoing an effective program to train students to be competent and
skillful in the qualification of Computer Systems Servicing NC II.
I further recommend that the workshop layout must be given focus to
conform to the components of CBT standards.
