GOAL:
To assess the fetal position, fetal lie, fetal
presentation, and fetal attitude.
1. Prepare the patient.
a. Explain the objectives of the procedure.
Secure consent and provide privacy.
b. Instruct her to empty her bladder.
c. Prepare the client: Instruct her to empty her
bladder, then position her to dorsal recumbent
then drape.
2. Wash hands.
3. Observe the woman's abdomen for longest
diameter and where fetal movement is
apparent.

4. Perform the first maneuver.


Objectives: To determine what occupies the fundus
of the uterus.
a. Stand at the foot of the client, facing her, and
place both hands flat on her abdomen.

b. Palpate the superior surface of the fundus.
Determine consistency shape and mobility, if it is soft
and irregular it is the buttocks, if it is hard and round it
is the head.
5. Perform the second maneuver
Objectives: To determine what occupies the left and
right sides of the abdomen, anterior or posterior.
a. Stand at the foot of the client, facing her,
and place both hands flat on her abdomen.

b. Palpate the side of the uterus. Hold the left hand


stationary on the left side of the uterus while the right
hand palpates the opposite side of the uterus from top
to bottom.

c. Then hold the right hand steady, and repeat


palpation using the left hand on the left side. If you
felt
a hard plane surface that is the back and if you felt
irregular nodules those are the extremities.

d. Determine the fetal position, lie and attitude.

6. Perform the third maneuver.


Objectives: determination of the presenting fetal part
in respect to the true pelvis.
a. Gently grasp the lower portion of the abdomen
just above the symphysis pubis between the
thumb and index finger together. Determine any
movement and whether the part is movable (not
engage) or not movable (engage).

7. Perform the fourth maneuver.


Objectives: To determine the location of the cephalic
prominence
a. Change your position facing the patient’s feet.

b. Place the finger on both sides of the uterus,


approximately 2 inches above the inguinal
ligaments, pressing downward and inward in the
direction of the birth canal. Allow finger to carry
downward to feel the cephalic prominence which is
the brow.
