What Happened:
On 15 August 2020 at around 14:39 hours, the laydown of 4” drill-pipe singles from the rig floor to catwalk
through the V-door using rig floor utility winches was being performed. During the lowering of the drill pipe (DP)
joints, the Roustabout (Injured Person – IP) placed the stopper pipes below the DP joint 25% from each end.
After the DP had been lowered onto the stopper pipe, the IP attempted to remove the lifting cap from the DP
joint. After disconnecting the winch line hook from the lifting cap, and during the removal of the lifting cap, the
DP joint slipped from the edge of the stopper pipe and pinched the IP right hand between the catwalk
structure and the DP. The IP did not sustain any open wound, however he developed mild swelling on his right
hand middle and index fingers for which he was provided with first aid treatment.

Results:

Flash Learning:
• Watch out for and guard against pinch points.
• Don’t place your hands under loads and places where they can get trapped.
• Remove lifting cap after the drill pipe has been rolled onto the pipe rack.
• Use mechanical tools/aids instead of hands wherever possible.
