A continuación, se presenta el Caso de estudio a través del cual realizara la actividad


solicitada para la Guía de Actividades de la Tarea 3.

CASE STUDY – AMPUTATION IN MEAT GRINDER


A 15-year-old part-time worker (the victim) at a retail grocery store suffered amputation
of the right arm as a result of being caught by the auger of a meat grinder while he was
reassembling it.
The victim, working after school, had completed washing and cleaning the
disassembled parts of the grinder and was reassembling it without de-energizing and
locking out the machine's power supply (See Photos Below). He inserted the auger into
the grinder's housing and reached through the feed-throat with his right hand to guide it
into engagement. As he did so, he bumped against the unprotected on/off lever switch
mounted about waist high on the machine's side.
The grinder started and the auger pulled his hand and arm into the housing. He turned
the machine off, pulled his arm from the grinder housing and ran toward the front of the
store. The store manager applied pressure to his bleeding arm while a coworker called
911. The victim was transported to a local hospital. His right arm had been amputated
just below the elbow. A magnetic safety switch mounted on top of the motor starter was
designed to prevent operation when the Removable Tub
was taken off. The safety switch had not worked for over 16 years and employees were
not aware that the safety switch existed. (Source:

A. Actividad:
Realice el análisis del accidente sucedido con el fin de hallar la causa raíz, utilice la
metodología de investigación de accidentes Árbol de causas o Diagrama de Ishikawa
para este análisis. Discuta con su grupo colaborativo para que cada integrante se
enfoque en una metodología diferente.

Realice el análisis del accidente sucedido con el fin de hallar la causa raíz, utilice una
de las siguientes metodologías de investigación de accidentes:

 What was the main cause of the incident?

Although it is evident in the Ishikawa diagram, there are some causes that end up having a

catastrophic effect for an employee, causing a serious work accident. There is a cause that

stands out, this reflects the main cause of the incident, the employee/victim decides to carry

out said cleaning of the machine (shredder) without first verifying that said machine was

connected to electricity. Even so, with his years of experience in the work he employed,

the employee had verified that the machine was disconnected from electricity, even if he

had tripped over the power switch, the machine would not have started its operation,

avoiding the loss of the right upper extremity.

 ¿Cuál fue la principal causa del incidente?

Si bien se evidencia en el diagrama de Ishikawa, se encuentran algunas causas que llegan

terminar un efecto catastrófico para un empleado causando un accidente de trabajo grave.

Hay una causa que resalta, esta refleja la causa principal del incidente, el empleado/victima

decide realizar dicha limpieza del maquina (trituradora) (“sin antes verificar que dicha

maquina estuviera conectada a la electricidad”). El empleado aun asi con sus años de

experiencia en la labor que empleaba, de haber verificado que la maquina estuviera

desconectada a la electricidad, por más que se hubiese tropezado con el interruptor de

encendido, la maquina no hubiese iniciado con su funcionamiento, evitándole la perdida de

la extremidad superior derecha.
