    AUTHORIZATION LETTER

    To concerned personnel,

    I am MELVILLE MAE A. BILAG, a resident of Agustin St, Zone 1, Villa, Sta.


    Teresita, Cagayan.

    I am writing this letter to inform your good office that I am physically unable to take the
    appurtenant pay-out which was the fruit of the 10 days tree planting conducted previously at
    Allilaw Cave, Sta. Teresita, Cagayan. The reason for my absence was because of the distant
    constraint, considering the risk of COVID-19 and ultimately for the sake of being a mother I
    have to take care of my son since no one will do it if I will leave him because of his tender age.

    NOW THEREFORE, I am respectfully authorizing MS. CRISTINA CABASAG to take


    the necessary pay out on my behalf.
