In relation to this, the student-researcher of Grade

11-Humanities and Social Sciences (HUMSS) will be conducting
a Qualitative research entitled “Effect of teachers student
relationship on students motivation” This project will be
conducted under the supervision of our Research teacher,
Mrs. Janice D. Rivas.

We are hereby seeking your consent to allow us to

conduct a survey in Mambusao National High School in order
for us to gather data we need in our study as part of the
requirements in the completion of our Research subject in
the SHS Curriculum.
