name, M.P. ALVERO AIRCON & REFRIGERATION SERVICES
located at 49A G. Araneta Avenue, Santol, Quezon City under
Mayors Permit No.20-008315;

That, the said business was decided to stop being a retailer

(Refrigeration Parts and Supplies and Aircondition Parts and
Supplies) and Other Business and Businesses with Fixed Rates
(Delivery Motorcycle) on April 4, 2022 due to change of line of
business;

That no income, revenue, or profit of any kind was derived

from said business;

That I am executing this affidavit in order to attest the truth of

the foregoing facts and for all legal intents it may serve.
