3. ALI GENDENG BUISAN is the name indicated in my most recent

passport;

4. I used the stated name as my alias in pursuit of or in furtherance of

my political beliefs;

5. The names MALIK MANTAWIL and ALI GENDENG BUISAN refer to

one and the same person;

6. That I am executing this affidavit to attest to the truthfulness of the

foregoing and clear the discrepancy that may arise from official
documents bearing the name MALIK MANTAWIL or ALI GENDENG
BUISAN, and for whatever legal purpose it may serve.
