AFFIDAVIT SOLO PARENT

I, SUZETTE SHAYNE S. AQUITAÑA , of legal age, Filipino and presently residing at

Barangay Magancina, Santa Barbara Iloilo, after having been duly sworn to in accordance with law
hereby depose and say ;

1. That I am a young mother of my (2) sons:

a.Brein Niñ o Aquitañ a 5 yrs old
b. Brevien Agustin Aquitañ a 2 yrs old

2. The two children are under my custody who are living with me and my family at our
residential home in Barangay Magancina, Santa. Barbara, Iloilo.

3. That I am a single parent but with the guidance and help of my family and up to present I
am solely providing for the needs of my (2) sons;

4. That I am executing this Affidavit to attest to the truth of the foregoing facts and for
whatever legal purpose and intent it may serve.
