AFFIDAVIT OF UNDERTAKING
I, Norma L. Sebastian, of legal age, Filipino, and a resident of 688F Road 1 , Brgy.
Matandang Balara , Quezon City , after being duly sworn to, under oath and in
accordance with law, do hereby depose and state that:

1. Sometime in year July 1992 , Winnie Baguis came to our house in Brgy. Old Balara to
sell a parcel of Land her family owned in Brgy. Aurora , Tarangan Samar. From then
my family attended and harvesting copra from the said parcel Land.

2. We agreed to the amount of P8,000 and pay her in cash and sign in a yellow
paper that she is selling the land and give me the copy of Land Title, we agreed
that she will come back to our house to sign a typewritten copy of our agreement
which she did not return.

3. The said signed paper was lost /totally damaged in my possession because my
house was totally wrecked by a tree that fell on our house because of Typhoon
Glenda last July of 2014.

4. THAT I exerted diligent efforts to locate or but to no avail and I therefore

considered the same as lost/damaged and can no longer be found;

5. I am executing this Affidavit to attest to the truth of the foregoing facts and to use
the same for whatever legal purpose it may serve.

WE are all residents of Road 1 , Bgry. Old Balara and attest that the house of
our neighbor NORMA L. SEBASTIAN was totally damaged DUE TO FALLING OF
