MEMORANDUM OF AGREEMENT

This Memorandum of Agreement made and entered into this 15th day of November at
Himamaylan City, Philippines by and between:

Agency of the nation government of the Philippines with the office address at Vallega St. Brgy. I
Poblacion Himamaylan City represented herein by its Schools Division Superintendent
GLADYS AMYLAINE D. SALES, as Schools Division Superintendent herein referred to as
” DepEd”.

EYE EAR Foundation Philippines Inc. is a national, non-stock, non-profit, organization

providing vision and hearing health interventions to all Filipinos, existing under the laws of the
Republic of the Philippines at an individual with the office address at 9,21 St.Extension,
Brgy.5, 6100,Bacolod City represented on this transaction by CHARITY FAITH BERJIT as
Program Manager by its herein after referred to as the” PARTNER”

WITNESSETH: THAT,

WHEREAS, Republic Act 8525, otherwise known as An Act Establishing the Adopt-A-School
Program and Providing Incentives, Therefore and for other purpose entered to encourage
government agency to assist in the delivery of better quality education to public schools in the
country, particularly in the provinces:

WHEREAS, Revenue Regulation No.10 s,2003, Implementing the Tax Incentives Provision of
RA 8525, otherwise known as the Adopt-A-School Act of 1998 has been issued by the
Department of Finance thru Bureau of Internal Revenue

WHEREAS, DepEd has the responsibility of securing resources to make our public schools
competitive and its now calling on the private sector to serve as a major partner in the nation’s
development and in the improvement of the public education system.

WHEREAS, DepEd believes that one way of achieving an enhanced learning environment and
addressing the needs of resources in public schools is through the provision of the Conduct of
Vision and Hearing Screening to Grade 1 to Grade 6 especially SPED leaners, teachers and
parents who would like to avail .

WHEREAS, the PARTNER prepare all necessary measures needed for the said activity based
on the safety and health standards to secure the health of their patients and of other persons
involve in the outreach proposed to DepEd by the EYE EAR Foundation Inc.

WHEREAS, the PARTNER is free screening for all. Ow 18-below only. However, the
prescription glasses is free for the children below 18-below only. For adults who may need
prescription glasses may be able to avail in minimal counterpart depending on their their eye
grade.

Memorandum of Agreement signed by the PARTIES and their instrumental witness, and they
acknowledged to me that the same is their free voluntary act and deed and that of the parties
they represent respectively. This Memorandum of Agreement consist of five (5) pages including
