February monitoring tool summary as reflected on the table above shows 100%

Table C shows the percentage of Learning Activity Sheets (LASs and Self-Learning
Modules (SLMs) that were printed, prepared, and distributed to learners. A ratio of 1:1
LASs/SLMs in all subject areas were prepared by each subject teacher for the month
of February.
Teachers records for SLM distribution and retrieval receipt were securely signed by
parents, hence parents’ regular collection and returns of SLMs were monitored weekly.
For the month of February, teachers’ data about modules receipts signed by parents
shows 100% result just like of the previous month. Constant reminder to parents in
getting the modules on the scheduled date and time were still observed so that
teachers were able to do their tasks as planned.
Data on Table F shows the 100% result of feedback provision for week 7 of quarter 2
and week 1 of quarter 3 except for Weeks 8 & 9 of quarter 2 since respectively, it was
end of the quarter and class break. Self-Learning Monitoring Tool is the key to which
teachers able to provide immediate feedback based on learners/parents’ concerns
regarding activities on modular instructions. Aside from SLMT, teachers do provide
specific feedbacks on learners’ outputs by utilizing the teacher-made feedback form or
directly on learners answer sheets. Upon provision on of feedbacks it was given back
to parents/learning facilitator for them to evaluate the notes given and gave direct
response. By giving feedbacks, it is evident then that the teachers have checked and
analyzed the learners’ output.
Based on the data presented in Table G, it shows that there were 59 queries out of 193
learners or 30% queries for teachers to answer for this month of February which was
94% lower compared to January result. Queries been answered by teachers through
their teacher-made query-response form. Then they gave it back to the parents so that
learners will have a guide in answering competencies which is unclear to them.
For the month of Februaury, teachers have checked the learning tasks and output of
the learners, with updated record in learners Individual Learning Monitoring Plan
(ILMP). To assist struggling learners or those who were falling behind in answering
their modules, teachers have made Personalized Weekly Home Learning Plan (PWHLP)
and provided remedial activities that suits to the level of the learners. Due to regular
monitoring of learners’ accomplishments of learning tasks and outputs, results of
insignificant level of progress for Second Quarter were prevented.
February 2022 Monitoring results shows 100% of teachers prepared Weekly Home
Learning Plan to serve as guide of every learning facilitator. These were printed and
distributed to the learners in 1:1 ratio. The same with ILMP, 100% of teachers
prepared Individual Learning Monitoring Plan to record learner’s output/performance
result. Those learners who were lagging-behind were provided with learning activity
sheets for which activities were simplified to suit to the level of learner’s learning
capability.
Table above shows 100% of teachers prepared summative tests. All grade level
prepared 1 summative test for the month of February and that is summative test
number 4 of quarter 2 which falls on week 7. Printing of the said test papers were 1:1
ratio. Before reproducing it, careful checking and editing takes place for which written,
and performance task must be evident at same time the congruency of tests to
learning competencies for the covered weeks.

Table above shows 100% of teachers prepared and submitted their IDLAR and WWP
for each week of February 2022. For this month, all teachers able to submit the
weekly required report on scheduled date, thus their efforts were acknowledged.
