1. Implemented the approved Supervisory Plan for September 2021 and peformed
instructional supervision to 3 teachers in the different learning areas under the MDL-
Print of the Distance Learning;
2. Conducted Classroom Observation using the PPST-COT for 3 teachers (Angela Luz
D. Del Rosario, Krisha Angelica Y. Calub, Jojo A. Cruz) through District Innovative
Project (Project GOALS)
3. Monitored the implementation of LDM- Print and learning intervention activities to
non-mastered skills/ competencies in all learning areas;
4. Monitored the implementation of the school best practices in improving pupils
performance towards higher learning outcomes;
5. Conceptualized impact study on the implementation of the school and district
innovations on improving the academic performance of the learners:
a. Project CHAMPION (Championing and Hurdling the Challenges on
Acquiring the Skills and Mastery in Promoting Innovative and Creative Works,
Research Works, IGP to Optimize the Teachers’ Professional Development
Opportunities as a noteworthy Project of Moncada North District)
b. Project REACH- “Radio-Based Instruction Learning Delivery to Reach All
Learners to Have Equal Access to Basic Education to Acquire the Needed
Competencies and Skills and to Help Remote Learners Hurdle the Challenges of
the New Normal in Education.
6. Provided Technical Assistance to teachers regarding the LDM- Print and Radio based
Instruction;
7. Monitored the distribution of modules to parents for the home learning of pupils.
8. Checked and monitored the implementation of the Weekly Home Learning Plan.
1. Monitored the conduct of the Monthly (September 2021) Classroom HPTA Meeting;
2. Conducted Kumustahan Activity with the GPTA Officers and SGC Officers;
3. Submitted Report on School Programs, Projects and Activities ;
4. Monitored the Continuous Improvement Plan (CIP) entitled
a. Project KINDNESS- Kindling the INnate Aptitude by Developing Skills
Development Project in Gardening through Mulberry as Noteworthy Project
Accessible Basic Education to Support Children in School.
b. Project DREAM – Delivery Worthwhile Project through Mulberry Farming to Raise
the Educational Academic Outputs and Mastery among Learners.
c. Project MASTER – Making All Financially At- Risk Learners Achieve Scholastic
Excellence Through Mulberry Project to Support the Educational Needs of the
Financially At- Risk Learners.
d. Project BRIDGE- BRidging Learners to Scholastics Success among Indigent
Learners by Developing their Skills in Mulberry Farming to Gain Financial
Augmentation in Finishing their Elementary Grades.
1. Submitted the Learning Continuity Plan of VES for the Approval of the SDO.
2. Utilized and submitted MOOE Liquidation Report for the month of September 2021.
3. Updated School Financial Status and Transparency Board;
4. Prepared Documents for SBM Evaluation;
5. Updated the School Based Management (SBM) Level of Practice;
