HSE & General Requirements for Working at Fatima Fertilizer Company Limited
1. Contractor shall abide by all site safety rules applicable to FFL Complex communicated through Safety Manual SAF-WP-IMS-203.
2. Contractor will be responsible to communicate Emergency Role Statement to his work force. Contractor to consult FFL Emergency Response
Procedure SAF-WP-IMS-108 for details. In case of Emergency;
2.1 Stop all jobs in a safe manner and vacate immediately.
2.2 The personnel shall report to nearby Assembly Point depending upon wind direction.
2.3 Contractor/Supervisor shall conduct head counting and report to FFL Contract Manager.
3. All contractors and workforce shall go through safety Induction & technical training related to Job Scope prior to start of work at site.
4. Contractor shall ensure that no worker is without Safety orientation & security passes. Company has the right to stop any worker or contractor to
execute job without valid security passes. ID cards shall be issued to only those workers who have completed safety Induction.
5. Contractors are bound to follow all the safety & environment requirements, communicated to them by Fatima Safety Staff and contract manager.
No job will be carried out without work permits i.e. Permit to work, Hot work permit, Confined space entry permit & Excavation work permit,
vehicle entry permit spill control plan (where applicable) etc.
6. Contractors shall provide safety gears (safety shoes, safety hat, working gloves, goggles and overall) as per FFL PPE standards. All workmen
shall follow Fatima's dress code (wear coverall). Loose clothing like Shalwar Qameez, Mufflers etc. are not allowed. No worker shall be allowed
to work without mandatory & advised safety gears.
7. All contractors who are involved in work at or above six feet height shall positively provide their man power Five-Point-Full-Body harness with
Double lanyards arrangements with complete fall protection.
8. All excavations shall be carried out on Excavation certificate with the prior consent of concerned area supervisor (in order to verify any
underground utility lines in area). All such excavated areas shall be cordoned off with a warning tape. Excavations deeper than 4 feet (1.22
Meter) shall be protected with suitable shoring. If the excavation is in the form of trench which is in depth of 4ft (1.2) or greater, shall be
considered as confined space and Confined space entry permit shall be applied for compliance. Contractor shall provide proper access and
egress in all excavations in deeper than 4ft (1.2m) by providing Aluminum ladder.
9. Contractor shall use all inspected portable electrical tools with the ELCB (earth leakage circuit breakers). No electrical equipment shall be used
at site without ELCB.
10. Contractor shall not make temporary electrical connections. All connections shall be equipped with proper sockets, plugs and ELCB if
applicable. Broken or damaged cables and electrical fittings shall not be allowed at site. Pedestal Fans of metallic material are not allowed at our
site.
11. Contractor shall have to get all his electric power appliances, tools, equipment and safety harnesses inspected from Fatima's workshops prior to
usage at Fatima complex and off sites. Voltage supply for all portable electrical tools and lighting shall not exceed 110 Volts.
12. For the jobs involving welding machines, the Contractor must submit Fitness Certificate of welding machines (fitness for all components
electrical/electronic components) to Contract Manager before mobilization to plant site and get it tested from FFL Workshop/Electrical team. In
case of any incident related to faulty, damaged, unhealthy or untested welding machine, suitable penalty will be imposed as per JO/Contract
penalty clause.
13. All rental cranes, vehicles & portable equipment (welding machines, jetting machines, generators etc.) must be inspected/ tested from FFL
workshop (motor pool/ electrical whichever applicable) prior to mobilization at job site.
14. Scaffolding shall be erected on foot plates, with cross bars, side braces, toe board, mid rail and hand rails with proper access to working
platform as per standard practice. All scaffolding structures shall be inspected by competent and trained supervisor and will be tagged in lieu
with FFL scaffolding tagging system.
15. Contractor shall ensure that no unhealthy, unskilled or untrained worker perform duty at plant site. This includes mandatory physical fitness and
safety induction at site.
16. All crane and vehicle operators must be certified and shall be in possession of valid driving license. A valid vehicle fitness certificate or third
party inspection certificate is mandatory for all the cranes and lifting equipment to be used at our site. Vehicle will be checked as per checklist &
emission test will be carriedout in Workshop by HSE team.
17. Traveling on industrial vehicles is not permitted. Contractor shall ensure that their personnel do not traveling on cranes, graders, rollers and fork
lifters etc.
18. Contractors shall take utmost care during hot works i.e. welding, cutting grinding etc, or any job that has potential to produce heat. Steel planks
and structures shall be used for hot jobs. Contractor shall ensure that fire blankets & inspected fire extinguisher are present during hot work
activity site.
19. Vessels and confined spaces entry shall not be made without issuance of entry permit. Contractors shall not attempt any kind of entry without
written approval of concerned Manager/supervisor.
20. Contractor shall keep a first aid box at worksite. In case of some serious injury contractor shall seek help from Fatima medical center, then
report incident to HSE dept. and concerned supervisor.
21. Contractor shall be responsible for the area housekeeping. Job will be considered incomplete until housekeeping is finished.
22. Contractor shall provide welfare facilities to its workers while working at our site.
23. Contractor shall depute well experienced & suitably qualified/ competent HSE supervisor for the committed jobs.
24. HSE supervisor shall be responsible for the overall execution of the job and shall also perform the following tasks;
24.1 Ensure compliance to FFL procedures mentioned in Contractor Safety Manual.
24.2 Record, report and take appropriate action in case of injury, fire and near miss.
24.3 Record and take corrective measures against any unsafe acts observed by self or FFL interfaces during site visits and Management
Safety Audit.
24.4 Ensure that daily Tool Box talk takes place and submit evidence to FFL contract manager.
24.5 Keep record of man-hours spent throughout the job.
25. Contractor's HSE Supervisor shall take part in incident investigation w.r.t. incident at Contractor's assigned jobs.
26. Contractor shall be bound to report all incidents regardless of type and severity to FFL representative immediately. On failure to breach the
compliance, Strict disciplinary action will be taken against the contractor which may can lead to cancellation of the contract depending of the
severity of incident.
27. Contractor will make to understand the Scope of work & their Risk assessment.
28. Contractor shall make sure safety signage in his assigned area.
29. Contractor shall follow FFL Mobile Phone Guidelines, Traffic and Driving Policy & Smoking policy.
30. Contractor shall follow FFL Cardinal Rues which are as below;
30.1 It is mandatory to report all "On the Job" Health, Safety & Environment incidents.
30.2 Disabling of Safety critical & Security devices, without proper written authorization, is strictly prohibited.
30.3 Lockout/Tag out & Try procedure for all isolations are mandatory.

30.4 Lighting a flame, without authorization, in Operating areas and Warehouses is prohibited
30.5 Engaging in or provoking horseplay or fighting within Company premises is prohibited
30.6 Smoking is not allowed in Plant & Office premises except designed Areas
30.7 Willful damage to Company property.
30.8 Violation of traffic rules within Plant and Colonies.
30.9 Any person under the influence of drugs or alcohol shall not be permitted to enter Plant & Office premises.
30.10 Weapons, intoxicants, alcoholic beverages and illicit drugs are strictly prohibited in plant and Office premises
30.11 It is mandatory to follow all Company policies, safety rules and applicable laws as stipulated by the government & the Company from time
to time.
30.12 Any violation of the above rules shall be treated as Misconduct.
31. Contractor's performance at work shall be measured as per Contractor Feed Back Form (P&C-25). Contractor shall conduct routine toolbox talk
and document the attendance. Toolbox talk summary form and Feedback form will be submitted with final invoice.
32. Contractor shall abide by all Environmental rules/Acts applicable to FFL.
33. Contractor to intimate P&C about job commencement date within 03 working days of JO issuance.
34. Penalties on violation of HSE rules, Environmental rules & Job quality will be imposed as follows;

SECURITY INSTRUCTIONS FOR CONTRACTORS
Objective:
The purpose of these security rules is to all contractors for performing work securely at Fatima Fertilizer Company Limited FFL. It outlines the rules
which the contractor are required to follow when undertaken work.
Scope:
The procedure applies throughout FFL site unites for Manpower Services, Rental Equipment, Inspection and consultancy Services.
These rules are contractual obligation on contractor especially critical contractors (i.e. all the contractors working inside plant battery limits and
working on product handling and storage area) who shall ensure that the rules and procedure are brought to the notice of all the contractor's
employees, and applied accordingly. The responsibility for ensuring that contractor's employees understand and comply with the relevant procedure
rests with the contractor himself.

Cardinal Security Rules.


1 All Contractors' Workforce must have valid CNIC.
2 All Contractors' Workforce must have valid company Identity Card and that card must be displayed on job all time
3 Security clearance of Contractors' Workforce deputed for more than 79 Days through concerned police Station / Nazim /Nambardar
4 Carrying of weapons is strictly prohibited.
5 Daily security checking at the time of entry in FFL premises.

General Security Rules:


1. Security is the first priority. In the event of any contractor's employees willfully failing to comply with such rules. FFL may demand contractor
to replace the person involved in or terminate the contract in case of repeated and / or serious violation of security
rules/producers/practice is observed.
2. All contractors personal must stay in their specified area of work and should not enter any other area of the plant without approval from FFL
Site in charge.
3. Contractor shall ensure the scrutiny of his workforce by FFL security Executive/ Supervisor.
4. Contractor is responsible for compliance of FFL security procedure.
5. After formal verbal warning. Penalty will be charged for subsequent violations as tabulated below.

16-The Contractor shall follow all safety rules and work instructions issued
by the Owner or in force in the Owner premises. The Contractor shall
strictly adhere to his workforce regarding policies of the company (Safety
& Health Environmental and Quality). The Contractor shall ensure to
fulfill his environmental obligations regarding job in line with
environmental policy of the Owner.

Medical Fitness Criteria


Annual/Long Term Contracts:-
Contractor shall ensure physical health of his workforce by engaging an MBBS doctor for health assessment. Following medical tests are minimum
requirements that need to be fulfilled by contractor for respective workforce being hired for annual/long term service contract.
• General physical examination by doctor
• Complete blood count
• Hbs Ag (Hepatitis B screening)
• Anti HCV (Hepatitis C Screening)
• Blood Sugar
• Chest Xray (If Required)

Short Term Contracts:-


Contractor shall ensure physical health of his manpower through below mentioned minimum requirements of health assessment and submit
undertaking of fulfillment of following criterion.
