I, CRISANTE L. MARUQUIN, of legal age, residing at #166 Sitio Rang-ay, Brgy. Baduang, Pagudpud,
Ilocos Norte and currently the coach of Sepak Takraw Girls Secondary with the following students:

Jyrynne Bea G. Bayani
Crissa Tiffany A. Faelmoca
Zyselle Faith D. Nabarte

That I, and my Chaperon, JAYCEL P. ANDRES, both teachers from Dumalneg National High School are
required to attend the Town Fiesta to be held on April 15 to 18, 2023.

I understand that they are undergoing training in preparation for the R1AA to be held on May 14 to 19,
2023 and they are not allowed to go out during the duration of the training.

I understand that taking care of our athletes is a great responsibility, that we cannot let other teachers
from other schools take this responsibility and that I decided to bring them home from April 15-18,
2023.

I freely assume the risk and permit our students to attend the Municipal Town Fiesta upon approval of
our SDS of the municipality’s request. Thus, our students will go home on April 15, 2023, and will come
back on April 22, 2023, after the Third Quarter Examinations
