    In order for us to pay you self-employed, please complete the below. We need to establish if you are
    under supervision direction and control before we can pay you self-employed.

    1. What is your trade?

    2. Can you be told how to carry out a particular job?

    3. Can the supervisor direct or control how you provide the Services?

    4. Does anybody oversee you on a daily basis?

    5. Can anybody onsite direct you to how the work is completed?

    6. Can you be moved from job to job?

    If you have any queries regarding these questions, please call or email us. However, we do need the
    form completed via email before payment.
