I, ZAYDA SERRANO DEL BARRIO, of legal age, Filipino, and a resident of

No. 50 –H Libertad St. Mauway Mandaluyong City, after having been duly sworn to in
accordance with law, hereby depose and state;

That my true and legal name is ZAYDA SERRANO DEL BARRIO, born April
22, 1968 at Daet, Canariñas Norte to spouses Benilda Serrano and Mario Avellana Del
Barrio.;

That in my Marriage Certificate and SSS ID my name was erroneously entered as

ZAIDA instead of ZAYDA the true and lawful name;

That ZAIDA SERRANO DEL BARRIO and ZAYDA SERRANO DEL

BARRIO belongs to me as one and the same person;

That the discrepancy was unintentional and was the product of typographical
error;

That I am executing this affidavit in order to attest the truth of the foregoing facts
and to request proper authorities to correct my name from ZAIDA to ZAYDA and for
all legal intents it may serve.
