TIME: 7:00 to 9:30 pm
COST: $20.00 per person
Refreshments will be provided.

Dress Code: Semi-Formal

Acceptable Attire: Dress shirt, tie, dress slacks, or dresses. A suit jacket is optional. No jeans or
sneakers. No low cut or exceptionally short dresses as per the dress code at HMS.
Bring socks/slippers if you plan to take off your shoes during the night.

We the parents/guardians of _______________________________________ understand that our

child will not be dropped off at HMS prior to 6:45 pm. (Both front doors will be open to allow
students entrance to the dance.) If you are picking your child up early, you must sign your
student out at the front door main entrance. Students will not be allowed to leave without a
parent signature. For security reasons, no adults other than chaperones will be permitted
beyond the front door entrance at any time during the dance. I understand the
ticket price is non-refundable.
