1 Unauthorized Work CTRA shall be reviewed if there is any changes in scope of work, site condition, or any 2 3 L Issuer

Proper access to the scaffold shall be provided & personnel shall not climb on scaffold
bars.
While utilizing portable ladders three-point contact shall be maintained at all times.
Adequate illumination, light intensity (300 Lux), shall be ensured by using Lux meter.

Dismantled or any materials shall be lowered to the ground and it shall not be stored on
the scaffold platform
Don’t throw or drop the objects from height to the ground.
Use buckets with rope for loose nuts, bolts and other materials.
All the openings on the scaffolding platforms shall be protected with toe boards (6 inch).
5 Collapse of Scaffold Ensure the ground condition is firm enough to erect a scaffold. 2 3 L Executor
Condition of scaffold materials shall be checked prior to use.
Scaffold shall be inspected as per KNPC approved checklist and certified by competent
personnel in every 7 days.
Signature by contractor and KNPC scaffold supervisor / engineer shall be provided on
the green tag.
Number of personnel and other materials on the scaffolding shall be restricted as per
scaffolding load bearing capacity.
Erection of scaffold shall be start from bottom to top and Dismantling shall be start from
top to bottom in orderly fashion.
Scaffolders and Scaffolding supervisors / Site supervisors shall audit on each shift of
scaffolding use in order to ensure the integrity of scaffold and ensure on gaps between
planks.
6 Electrocution Proper function of ELCB (30mA) shall be ensured before start the job. 2 3 L Executor
Don’t use cable with multiple and loose joints.
7 Personal Injury due to Minimum personnel shall be involved in the job. 2 3 L Executor
slip, trip & falls The materials / electrical cables / Hoses shall be laid properly.
8 Heat Stress Drink sufficient cold water. 2 3 L Issuer
Modify work schedules and arrange frequent rest periods with water breaks in shaded Executor
areas.
If the worker is not alert or seems confused, this may be a heat stroke. CALL 888
IMMEDIATELY, Move the worker to a cooler/shaded area and apply ice as soon as
possible.
9 Night Work hazards Avoid working alone and always maintain the proper communications with coworkers.
Sufficient illumination shall be ensured in the work area. 2 3 L Executor
Safe path to/from work location shall be ensured.
Barrication and Flashing lights shall be ensured in the excavated area.

Provision of retro-reflective stripes on FR Coverall must be ensured.
Provision of retro-reflective tape on equipment shall be ensured.
Don’t work in a dark location to avoid reptile and insect bites.
10 Poor hazard Conduct Safety talk before starting the job and explain all precautions in language 2 3 L Executor
communication craftsman can understand.
Explain emergency evacuation procedures to ensure workers can escape easily in case of
emergency. (Follow the wind sock cross/up wind direction to reach the nearest transfer
point).
Ensure that workforce has understood all the precautions properly.
We agree with the residual risk levels and control measures required to perform the tasks based on the identified hazards. We confirm that we, the members of this
CTRA team have visited the job site to identify and document all the hazards associated with the activity.

Notes:
1. CTRA contains only those hazards and controls which are not captured in SWP Checklist and which could not be captured in Work Permits
2. Only competent (knowledgeable, skilled, trained & experienced) personnel {personnel who are competent to do this critical work} shall carry out this
critical work; This CTRA becomes invalid if this requirement could not be met
3. Implementation of all controls specified in Work Permits, SWP Checklist and CTRA is mandatory to ensure safe completion of this critical work
4. CTRA reference number shall appear in all pages of CTRA
5. Master Copy (original) of CTRA shall be retained in file for records and future reference
6. Photo copy of CTRA shall be attached with work permit

9. If same Critical Work is planned, then a new CTRA is mandatory; however previous CTRA may be referred to
