JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, BERNARD P. LAPPAY and ARONOLFO D. TAMBAOAN, of legal age, Filipino,

single/married/widow/er and presently residing at DUROC ANGADANAN ISABELA, both
after being duly sworn to in accordance with law, depose and state that:

1. We have personally known the person of PRIMITIVA T. NGARANGAD for a

long period of time;
2. We know for a fact that the person by the name of PRIMITIVA T.
NGARANGAD, appearing in some documents and records and the name
PRIMITIVA PALATTAO TAMBAUAN, as appearing in her Certificate of Live
Birth, refer to one and the same person and that her true and correct name is
PRIMITIVA T, NGARANGAD;
3. As such, we execute this Affidavit to attest to the truth of the foregoing
facts and for whatever legal purpose that this Affidavit may serve.
