My clinical site treats very few CSIs in the photon practice.

Most of these patients are


treated with protons. I have not had my rotation through that department yet, but I can
only image how great the plans turn out.

My first attempt at planning this CSI was to combine two planning approaches with
photons; forward planning on the brain (3D, 2 laterals) and inverse planning (IMRT, 4
posterior arcs on the upper and lower spine). This was my clinic's approach to treating
CSIs before proton technology came along. I did not show my plan to a physician at any
point.

After I completed my first plan my ProKnow score was a C and not much room for
improvement. The constraints in the brain were impossible to meet with the 3D plan, so
I changed that to an IMRT plan and was able to achieve an A in ProKnow. My preceptor
recommended 4 full IMRT arcs for the brain plan.

My second attempt at planning this CSI was no means perfect, but I believed it was an
improvement over the initial plan. Id like to think that I would not treat this plan
clinically. I'm sure a proton plan would put mine to shame. Proton DVH's look beautiful!
I cant wait to learn!!
