EXECUTIVE SUMMARY
The main objective of The Project is to provide free medical services to the people of
Barangay Centro 7. People who reside from the Barangay have various needs to sustain their
life. Most of their job is in lined with blue collar jobs where in they were exposed to different
elements. So in related to this, the organization of The Project enters to promote holistic care
for them.
The first part of the proposal is to coordinate with the Partners for necessary reminders and
standards for medical mission. The organization will provide health teachings to the people of
Centro 7 for them to gain information and to avoid unnecessary microorganisms that may
grow during their everyday lifestyle.
The second part is that, the organization will provide free medical consultations with the
cooperation and partnership to the RHU. The organization will also give free sanitary kits for
them to use during their everyday work.
The requirement of this program is to utilized a good intervention to this proposal.

PROJECT DESCRIPTION
Project Background
The Project is focused on objectives and goals in a certain through interviewing the
Residents of the said barangay in order to attain good construction of overall approaches,
hence it could give them more details and they can accommodate those who are needed and
the possible problems of the barangay.
During the survey in the barangay Centro 7, they found out that most of the
respondents have blue collar job, (Tricycle driver, vendors and construction workers). This
job is prone to bacteria and germs and other illnesses coming from the polluted area
especially their houses are near the drainage systems. They saw some children playing under
the sun. They are also playing near canals which can cause serious illness.
The Proponents chose to perform medical mission and provide free sanitary kits. Both
are important endeavors that can make a significant difference in people lives. Medical
missions involve a group of health professionals and volunteers traveling to underserved
areas or communities with limited access to health care services. These missions provide
medical care, health education, and support to individuals who may otherwise not receive an
adequate health care. Combining medical missions with the distribution of free sanitary kits
can create a comprehensive approach to health care, addressing both immediate medical
needs and long-term wellness considerations. In addition to providing medical services, your
team can educate individuals on personal hygiene practices, menstrual health, and proper

usage of sanitary products. This holistic approach can contribute to overall community well-
being and empower individuals to take charge of their health.
The Project must be accomplished by June 9, 2023. Preparation of materials and
equipment shall be performed for 4 days from June 4, 2023 to June 7, 2023 to be given
enough time for scouting.
The Project shall occur on the Day of the Medical Mission, June 9, 2023. It will start
early in the morning.
The Project shall be Partnered by RHU APARRI who shall give free medical services
and free sanitary kits to the community to prevent health illnesses.
Objectives
Main Objective:
The project aims to give free medical services to the people of Barangay Centro 7. Because
the people residing in the Barangay have various needs to sustain their lives, our organization
are entering into an engagement with the Residents of Alameda to elevate their health status
specially those of Blue-Collar Jobs and Children.
Specific Objectives:
● To coordinate with the Partners for necessary reminders and standards for the medical
mission.
● To prepare the necessary materials and equipment for the medical mission such as:
● For the Free medical Services and Consultation: Medical Equipment dependent on
what the Partner medical institution would require.

○ For the Free Sanitary Kit: Sanitary Kits contents such as alcohols, soap,
toothbrush and toothpaste, tissue and others
● To Assist in Medical Consultations, Medicine consultation, and Health Monitoring
● To Check and Assess the Vital Signs of the Residents of Barangay Centro 7
● To Provide Health education to the Residents of Barangay Centro 7 most specially the
children
● To distribute free sanitary kits to individuals for their health care.
● To provide free health check-ups every month even after performing the project to see
their health improvements.

o Phase 1: The seminar will be conducted at the Aparri Municipal Function Hall
o Phase 2: The preparation for the materials and visual aids will be at Lazza's residence,
Gaddang, Aparri, Cagayan
o Phase 3-5: These phases will occur at Centro 7 Barangay Hall.

About the Organization
The Organization who shall implement this Project are Freshman Nursing Students taking the
NSTP Course. As student Nurses, they aim to elevate the health of individuals.

: The knowledge, skills, and abilities required for a successful project will depend on the
project's scope and requirements. However, some common skills and abilities that project
teams may require include:

Project Partner
The Project will be Partnered by RHU Aparri - who will administer the free medical care,
consultation and drugs. They will be responsible for doing the physical examination and
documenting it. The patient will receive their advice from them regarding what to do
regarding their health.
Institutions Impacted
The Beneficiary of the said Project will be Barangay Centro 07, Aparri, Cagayan. The
Barangay shall be receiving free medical services. It is headed by Hon. Edgardo G. Alameda.

Impact on National Development
The exposure and knowledge of issues are increased by community engagement, which also
gives communities the opportunity to influence decisions that have an impact on their daily
lives, communities, and towns, cities, and neighborhoods.
It gives local Residents the chance to participate in public decision-making processes while
also enlightening and educating locals on political matters that have an impact on their day-
to-day lives. Community engagement encourages government and public decision-making
bodies to listen through feedback, which in turn shows the value of community involvement.
Because of this, relationships between public agencies and communities become stronger,
deeper, and more trustworthy as a result of community participation.

PROJECT WORK PLAN
The Project is a Hybrid Medical Mission which involves free consultations, Medications,
Health Education and Sanitary Kits distribution to the Residents of Barangay Centro 7. It
shall be divided into 6 phases: two of which shall be for preparation, 3 for the Program proper
and 1 for Documentation, Monitoring and Assessment. Each phase shall have a detailed plan
in relation to the project’s specific Objectives, thus aiming to achieve the completion of The
Project Goal.
Phases of the Project
Phase 1: Seminar with the Partners
Objective:
To coordinate with the Partners for necessary reminders and standards for the medical
mission.
Summary:
In this preliminary Phase, The proponents of The Project shall coordinate with the
Principal Partner of the project, The Rural Health Unit of Aparri. As Student nurses, they
shall undergo a type of seminar for them to be educated of what shall happen in the said
Medical Mission and of what Nursing skills and knowledge shall they be taught of to make
the Project successful. Other reminders such as necessary equipment and materials shall
also be tackled at this phase. The said agenda of the Seminar shall be prepared by the RHU
of Aparri themselves.

Timeline:
Phase 1 must be accomplished for a period of three days from Thursday, June 1, 2023 to
Saturday, June 3, 2023.
People Involved:
The people who shall be involved in the seminar shall be the Proponents of the project and
the Authorities of the Aparri RHU who shall be conducting the said seminar.
Resources and Materials needed:
Although Visual aids and other educational materials shall, most probably, be prepared by
the RHU of Aparri, The proponents of The project are expected to bring their own packed
lunches, Hydration and other necessary things to allow an undisturbed flow of the seminar.
Detailed Strategy of the Phase:
The said program of seminar is meant to educate the Nursing students on how the
upcoming Medical Mission shall be conducted. The flow of the program would most likely
be prepared by the administration of the RHU Aparri yet the following Topics are expected
to be tackled from the seminar:
● The necessary Nursing skills needed for the program
● Nursing Data Collection
● The materials and equipment needed for the medical mission
● Preferred topic for the Health Education
● Contents of the Sanitary kit

● Distribution of Work
● And other necessary topics to bring the project to a success
Phase 2: Preparation of Materials and Educational visual aids
Objective:
● To prepare the necessary materials and equipment for the medical mission such as:
o For the Free medical Services and Consultation: Medical Equipment
dependent on what the Partner medical institution would require.
o For the Health Education: Visual Aids and Handouts
o For the Free Sanitary Kit: Sanitary Kits contents such as alcohols, soap,
toothbrush and toothpaste, tissue and others
Summary:
In this phase, the necessary materials are prepared before the project proper. Medical
Equipment shall be provided by the proponents of the project if possible and other
equipment shall be taken to consideration through borrowing depending on what necessary
equipment shall be required of as stated in the previous phase.
Timeline:
Preparation of materials and equipment shall be performed for 4 days from June 4, 2023 to
June 7, 2023 to be given enough time for scouting.

People Involved:
The said phase shall be performed by the proponents of the project, with the assistance and
consultation of the Aparri RHU Administrators.
The student will provide their own vital signs checking and monitoring apparatuses. Other
Materials needed for the medical consultation may be under the care of the Aparri RHU,
equipment which shall be dubbed as borrowed.
Each proponent shall also provide their basic personal protective equipment such as masks
and gloves. They will provide three boxes of Gloves and 5 bundles of surgical.
For the Health Education, the proponents shall prepare a PowerPoint presentation which
shall be fun and interactive to attract the interest of children with the barangay. The
proponents shall bring their own laptops and projectors.
The Sanitary kits shall be packed with the following items:
● Bar Soap
● Shampoo
● Comb
● Bandages
● Sanitizers
● Nail clippers

● Sanitary Pads
● Toothpaste
● Toothbrush
A Total of 210 Sanitary kits must be prepared for all the Residents of Barangay Centro 7.
This is to provide 3 Sanitary kits per household and with spare kits.
Receipt sheets shall also be prepared for the Free Medication services.
Since this would involve the use of money, the Project treasurer, Ms. Kria Lazza shall be in
charge of the releasing of the project funds. Each proponent of the project shall purchase
the needed contents for the project at Savemore Aparri.
Mr. Steven Kyle Palapuz shall be the one tasked to produce a PowerPoint presentation for
health education. The group researcher, Ms. Janelle Labuguen shall be the one in charge to
format the content of the Presentation.
Phase 3: Free Medical Mission
Objective:
● To Assist in Medical Consultations, Medicine consultation, and Health Monitoring
● To Check and Assess the Vital Signs of the Residents of Barangay Centro 7
Summary:
The student nurses shall make themselves readily available in assisting in the free medical
Consolations. They shall use the skills they learned from the Nursing program and the

Seminar they attended. They shall also be taking Vital Signs and Monitor the health status
of the Clients.
Timeline:
Phase 3 of the Project shall occur on the Day of the Medical Mission, June 9, 2023. It will
start at 8:00 AM and may last the whole day.
People Involved:
The People involved in the project shall be the The project Proponents, The Centro 7
Barangay officials and the Administrators of the Aparri RHU. The Beneficiaries of this
phase shall be the Residents of Barangay Centro 7.
The resources mentioned in Phase 2 shall be the relevant equipment and materials needed
for Phase 3 specifically the ones needed for the medical consultation. The Self provided
Vital Signs Checking apparatuses shall be used. Additional equipment needed shall be
informed to the proponents of The project at the Seminar during Phase 1.
The Nursing Students shall assist in whatever services the RHU shall conduct both as a
part of their Community Engagement and Skills enhancement. As a common protocol, the
proponents shall check the Vital signs of the beneficiaries of the project before every
consultation.
Phase 4: Health Education
Objective:
● To Provide Health education to the Residents of Barangay Centro 7 most specially

the children
Summary:
This Phase during the duration of The Project aims to educate the people of Centro 7
specially the youth. It is essential to teach the children at a young age to instill in them
knowledge regarding Sanitation. since at this age, they are most likely exposed to bacteria
during play, it is relevant to give them health education. Although this phase is focused to
teach the young, it is not limited to them but rather an initiative to be done for every
individual in the barangay, even adults.
Timeline:
The same day of the implementation of The project, June 9, 2023
People Involved:
The people involved shall be the proponents, the Barangay Officials, The RHU
administration and the Beneficiaries of the Project most specially the children of Barangay
Centro 7.
With the help of the RHU, there will be a visual presentation about health education
specifically about sanitation. The Youth shall be taught proper hand washing during the
course of the Health education. Interactive games shall also be done to spark the interest of
the Children and at the same time, entertain them.
Phase 5: Sanitary Kits Distribution
Objective:
● To distribute free sanitary kits to individuals for their health care.

Summary:
In this phase, the proponents shall distribute the 3 kits per household with spare kits to the
Residents of Barangay Centro 07. All of the proponents of this project shall participate in
the said distribution.
Timeline:
Distribution of kits shall be distributed after health education.

check up every month. They shall use the skills they learned from the Nursing program and
the Seminar they attended.

Project Budget
For the whole Project, it will be funded by the Proponents themselves. The following factors
are considered for the budgeting and implementation of the project.
Equipment Sanitary Kits
These are the basic and main supplies for the project proposal that will be distributed to the
target beneficiaries and will be use for the sanitary process.
Food Expenses
These are the secondary items that will be use for the project which also will be distributed
and is mainly for the consumption of the beneficiaries after the sanitation process.
Travel Expenses
These expenses will be mainly use for the transportation of the primary supplies and
secondary items from school to the beneficiaries' community. This will also means transit for

Even after the success of the program proper of project THE PROJECT, it is relevant that the
proponents must still monitor the health of the beneficiaries of barangay Centro 7. That is
why THE PROJECT team will do monthly vital signs to each and every one resides in the
barangay. They will provide a monthly consultation in the barangay. Vital Signs shall be
regularly implemented to monitor the health of the residents of Barangay Centro 7.

CONCLUSIONS AND RECOMMENDATIONS
Recommendations
To allow the Continuity of the project specially when it becomes a success, The Proponents
recommend that the project must be adapted by the College of Health to conduct it yearly. It
shall become an activity for Nursing Students taking NSTP as a Related Learning
Experience. This shall not only improve their nursing skills but also their community
engagement skills.
Conclusion
The project shall be a great help for the people of Barangay Centro 7. The Laborers of the
Barangay shall receive a sense of recognition, acknowledging their efforts for themselves and
