I hereby grant my full and unconditional consent for my child to participate in the
Peñafrancia Voyadores Festival Street and Pilgrims Dance Competition,
scheduled to take place on September 14, 2023 at Plaza Quezon organized by the
City Government of Naga.

I understand and acknowledge that my child's participation will involve physical

activities, rehearsals, and performances related to the festival, and I am fully aware
of the potential risks associated with such activities. I assure the organizers that my
child is physically fit to engage in the required activities and that any medical
conditions, allergies, or special needs have been disclosed to the festival organizers.

I hereby authorize the festival organizers and their designated trainers to enlist my
child's name in the entry form for the Peñafrancia Voyadores Festival. I also grant
the designated trainer the authority to sign and receive, on behalf of my child, any
participation cash assistance provided by the City Government as well as any prizes
in the event that my child's team or group emerges as winners of any category.

I understand that the participation cash assistance and prizes, if any, will be used for
the benefit of my child and the team/group, and I waive any claims or demands
related to the distribution of these funds and prizes. I further acknowledge that the
decision of the festival organizers regarding the awarding of prizes is final and
binding.

This consent is valid for the duration of the festival and any related activities,
including rehearsals, training sessions, and performances.

I have read and understood the terms and conditions outlined in this consent form,
and I willingly grant my consent for my child's participation.
