concluded this agreement regarding

The alteration of the relations and the maintenance of their minor children

I.
Preamble

1. The parties to this contract are parents of the minors: name, date of birth, address

2. In regard to the above the father and mother wish to conclude this Contract and its
purpose is to alter their mutual rights and obligations to their minor children from joint
to shared custody (50 / 50) under the Czech law as the Czech Republic is an habitual
residence of the children.

3. The father and mother agree as part of this action, to live apart and not molest each
other both having a right to our privacy free from intrusion and harassment by the
other party.

II.
Agreement about custody and contacts

4. The father and mother agree not to plan activities for the children in the other parents
contact time without consent of the parent who has the contact time. e.g. parties,
sleepovers, school events, days out.

III.
Holiday time

1. Both father and mother respect the children’s right to quality holiday time and the
requirement to plan and book holidays in advance.

2. To facilitate that, the father and mother agree to support:

a. Two consecutive weeks with father and two consecutive weeks with mother
during summer holidays.

b. Eastr, Christmas, Autumn, Spring…

IV.
Unforeseen circumstances

1. In the event of one parent being unable to look after the children for their agreed time
(e.g. due to a business trip or illness) they will commit to being flexible and care for
the children wherever possible between the father and mother.

V.
Schooling

What school the children will attaned, who pays for fees.

VI.
Agreement about the maintenance

1. The father is obliged to contribute to the maintenance of minor children starting legal
force of that judgment the amount of CZK per month payable within every 10th of the
month, for the attention of the mother.

2. The mother is obliged to contribute to the maintenance of minor children starting legal
force of the judgment amount of CZK per month payable within every 10th of the
month, attention father.

3. In summary the father will transfer CZK to the mother on the 10th of each month.

VII.
Obligation to inform

1. The father and mother undertake that they will inform each other about all
circumstances relating to minors’ state of health, upbringing, location and education.

2. Father and mother agree to exchange contact details including mobile phone number,
email and postal address, which they will keep up to date to enable them to contact
each other in relation to any matters in relation to the children. In addition to discuss
circumstances affecting agreement arrangements have changed.

3. The father and mother are obligated to prepare properly the minors for the time with
other parent and not to disturb the time spent with other parent.

4. The father and mother agree that with regard to cultural and national origin of
children, both parents will mutually respect other parent’s effort to hand over them
culture of their country.

5. The father and mother will inform each other about important events of their children
well in advance so that both of them can be present at the event.

VIII.
Travelling abroad

1. Father and mother must consent to children being taken abroad, for a maximum of 2
consecutive weeks and travelling abroad cannot happen at a time they should be
having contact, unless it’s by agreement.

2. The father and mother agree to inform each other 3 months advance (where possible)
about travelling abroad including providing: flight details, who is going, where staying
and emergency contact number. For travel purpose the parent, who has the minors’
passports, is responsible for checking their validity and will hand over passports to the
other parent 28 days in advance (unless being used).

IX.
Decision making

1. The father and mother are obligated to make decisions on important issues relating to
minors together and in their best interest. The father and mother are obligated to
inform each other about minors’ state of health, education, extracurricular activities,
residence and close people, whom the other parent lives with. The father and mother
are obligated to communicate to each other important contacts to people such as
doctors, teachers and family members.

2. The father and mother agreed that both parents are able and capable of custody and
there is no need to doubt it.

X.
Obligation

1. The father and mother agreed to validate this agreement by the court. Both parties will
ask Obvodni soud pro Prahu within 3 days from the signature to validate the
agreement in form of court decision.

2. The contract is valid once signed by the father and mother.

XI.
Final provisions

1. The father and mother are declaring they have read and understood the content of this
Contract and that they have not concluded it under duress or prominently
disadvantageous conditions, and their signatures are the evidence to it. The agreement
has been supervised by lawyers of both parties.

2. The Contract was drawn up both in Czech and English and the English version is
determinative. Both parties comprehend the English version.

3. The Contract has three identical copies and each party will receive one copy.

4. Rights and obligations not regulated in this Contract are governed by valid the Czech
law, particularly by the civil law.
