CHANGE OF NAME CERTIFICATE

The service is mainly used as the preliminary application for procedure of Name
change. The citizen would be applying for the Name Change through Meeseva for the
preliminary verification which is done by Tahsildar .In the Process of Change of Name
the Annexure-II will be issued on the request of the applicant and on production of the
record to the Tahsildar.

Apply for Change of Name Application :


This section contains instructions for the operators of the Meeseva web application to apply for
Change of Name Application .
1) In Meeseva home screen, under List of Services, select Revenue Department services as
depicted in Figure 1.

2) Select Change of Name Certificate Service in Revenue Department as depicted in Figure

3) Now Change of Name Application Request Screen will be displayed as depicted in Figure 3.

4) Select Type of Application Either Minor/Major as depicted in Figure 4.

Note:- Here I am selecting Application Type as Major.

5) Based on Application Type Request details page displayed as depicted in Figure 5.

6) Enter all the Applicant details such as Applicant Present Sur Name, Applicant Proposed Sur
Name, Applicant Present Name, Applicant Proposed , Gender, Marital Status Husband/Wife
Name, Father Name, Mother Name, Police Station Name having jurisdiction, Age, Place Of
Birth, District, Mandal, Village, Locality/Landmark, Door NO and Pin code as depicted in Figure
6.

7) Select Profession from drop down as depicted in figure 7.

Note: - Here Employee details are displayed Based on Profession.

8) Enter all Employee details such as Name of the Employee, Name of the Company,
Designation, Date Of Appointment and Company Address as depicted in Figure 8.

9) Enter all the Informant details such as informant Name, Relation with Applicant, Mobile No,
Email Id and select Delivery type. As depicted in Figure 9.

Note: Based on the Citizen request, the delivery option (either through Manual or Speed Post -
Local or Speed Post - Non Local at the Franchisee) has to be selected.
The courier charge for Post (Local) is INR. 33/- and for Post to Non Local areas is INR. 46/-.

10) Enter all the Postal Address such as District, Mandal, Village/Ward No, Locality/Land Mark,
Door No/H. No and Pin Code as depicted in Figure 10.

11) Collect the necessary supporting documents as indicated in the Change of Name
Application Certificate request service page scan the documents and upload into the system,
as depicted in Figure 11.

Note: All the mandatory fields have to be filled in; otherwise request will not be accepted by the
system.
Note: Ensure that the Document Address of the Land is accurate as the concerned mandal
officials visit Permanent Address during the verification of his/her Change of Name particulars.

12) Click Show Payment. Another window appears requesting to Confirm Payment as
depicted in figure 12.

13) Based on the delivery option chosen by the citizen, the service charges will be calculated
and displayed in Show Payment as depicted in figure 14.

14) Collect the amount from the Citizen and click Confirm Payment to submit the request as
depicted in figure 15.

On confirmation, a receipt will be generated as depicted in the following figure. The receipt will
contain the Certificate delivery date.
Note: Ensure that you have sufficient balance with your service provider (SCA), else the
system will not accept the request.

Note: - On submission of the request, Citizen Receives Message. Like your Request for Change
of Name Application has been entered vide Application Number, Transaction Id and sent to
Dept.
15) On submission, the request will be sent to the work flow of the respective Tahasildar for
further processing.

16) Once the request is approved by Tahasildar, the Change of Name Certificate will be
dispatched through courier to the citizen address if the Delivery Type is Speed Post Local/Non
Local.

 If Delivery Type Is Manual, then the citizen need to collect the Change of Name
Certificate from franchisee where he/she applied for the Certificate.
